Reporting an Injury

Guidelines for Trainers

Player safety is our top priority. All injuries must be documented properly to ensure the well-being of our athletes and to support potential insurance claims.


📋 Medical Information Sheets

  • Every player must have a completed Medical Information Sheet on file.

  • Trainers must keep these sheets accessible at all times — during games, practices, and tournaments.


🩹 Injury Reporting Checklist

For a quick visual reference, trainers can use the OWHA “What to Do When There Is an Injury” guide.

This one-page checklist outlines the key steps for completing and submitting the OWHA Injury Form, including required timelines and contact information for assistance.

Key reminders from the guide:

  • Complete all sections of the OWHA Injury Form, including health insurance details.

  • Submit forms to the OWHA within 90 days of the incident.

  • Original receipts must be mailed to the OWHA office within 52 weeks of the accident for reimbursement.

🩹 When to Complete a Hockey Canada Insurance Form

Submit the Hockey Canada Insurance Form if:

  • A player or team official is injured during a game and misses more than one period.

  • A player or team official is injured during a practice or any other sanctioned event and requires medical attention.

  • A player or team official reports an injury after the fact (for example, they finished the game but sought medical attention later).


? Timely Reporting

  • Forms must be completed as soon as possible to ensure insurance claims are processed correctly.

  • Email a copy of the completed form to erica@pcwave.ca 

  • Keep a copy of all documents for your own records.


📈 Statistical Reporting

Send a copy of the completed injury report to the Ontario Women’s Hockey Association (OWHA) at insurance@owha.on.ca 

These reports are used for tracking and improving player safety across the sport.


🔒 Confidentiality

All injury and personal information is strictly confidential. It should never be used to evaluate a player’s ability to continue participating in hockey.


👨?👩?👧 Injured Player’s Parents or Guardians

  • Provide the partially completed Canadian Hockey Injury Report to the player’s parent or guardian.

  • Explain that this form is required to submit an insurance claim for medical expenses not covered by OHIP or their personal insurance.

  • Parents/guardians must mail the completed form and original receipts to OWHA within 90 days.
